Abstract

This surgical video presentation showcases a challenging case of an immature cataract with unexpected zonular weakness leading to zonular dialysis encountered during surgery. A comprehensive approach was employed in managing this complex situation. Initially nucleus was successfully managed by emulsification. To restore capsular stability, a combination of a capsular tension ring and a Cionni capsular tension segment was meticulously implanted. To address vitreous disturbance and ensure a clear visual axis, anterior vitrectomy was performed. Finally ,toric IOL was implanted to correct astigmatism and enhance visual outcome for patient. The video illustrates the step-by-step procedure, providing insights into the surgical techniques used to achieve this critical goal. This serves as a valuable resource for surgeons, offering insights into the management of such intraop complication, emphasizing the importance of adaptability and a multidisciplinary approach to deliver best outcome.
