Study design: Case report
Purpose: To report the rare ocular complication of orbital cellulitis with scleral necrosis in dengue.
Case: A 24 yr male with history of fever, malaise an body ache from last 5 days.No history of skin rash or bleeding from any site.He was diagnosed as DENGUE based on clinical feature and blood serology report positive for NS1 antigen.There was No perception of light in both eye.Both eyelids were edematous with periocular ecchymosis and severe tense proptosis.Extraocular movements were limited in all gazes. Bscan showed single mounds of hypereflective echoes suggestive of pre retinal haemorrhage.
Conclusion:Firstly, possible causes of hemorrhage could be thrombocytopenia with coagulation defects, capillary fragility, and platelet dysfunction.Secondly, the result of scleral ischemia secondary to the immune mediated vasculitis or due to coagulopathy associated with dengue hemorrhagic fever. This ocular manifestation of dengue is rare and carries poor prognosis.
