FP2244 : 'Peripheral dye rimming': a novel finding noted on CT-DCG in lacrimal sac tumor involving the stroma

Lacrimal sac tumors are rare, but clinically misdiagnosed as mucocoele, inflammatory granuloma. CT augmented by dacryocystography (DCG) is important adjunct to accurate diagnosis. While most epithelial tumors obliterate and distort lumen early, stromal tumors maintain lumen for longer period as a potential space.
27 years woman with left lacrimal mass since 4 years had ROPLAS-negative, probing showed firm stop. CTDCG showed well defined homogenous mass, not separate from lacrimal sac extending to a dilated NLD, with peripheral rim of dye noted along the medial tumor wall.
Excision biopsy was done. HPE showed intact stratified columnar epithelium with goblet cells and cells loosely arranged in the stroma in a spindle pattern, IHC confirming solitary fibrous tumor (SFT)
Lesions involving lacrimal sac area pose diagnostic challenge, novel finding like 'peripheral dye rimming' on CTDCG can identify stromal tumors prior to excision, aiding in appropriate management
