Abstract

50 Year old lady with no comorbidities presented with decrease in vision in left eye for 4 months.Her best corrected visual acuity was 6/6 in right eye and 6/9 in left eye with normal anterior segment examination.Right eye showed subretinal lesions suggestive of drusen.Left eye showed subretinal exudates with PED. She was provisionally diagnosed as having CNVM in left eye.After FFA ICG and OCT, she received one dose of intravitreal Anti-VEGF injection in left eye to which she showed significant improvement in visual acuity and almost complete resolution of exudate.Right eye continued to progress with increased subretinal lesions and exudate.Clinical diagnosis changed to primary vitreoretinal lymphoma.She underwent diagnostic vitrectomy.Subretinal fluid cytology showed features of intraocular lymphoma.Patient was treated successfully further with Rituxizumab in both eyes with chemotherapy. This is first case report of primary vitreoretinal lymphoma responding to intravitreal anti VEGF.
