Abstract

Purpose: To describe management of a case of Fulminant post intravitreal injection Endophthalmitis (FPIIE), presenting as Frosted Branch Angitis (FBA).

Clinical summary: A 42-year-old female, reported with blurring of vision in Right eye for 2 weeks, K/C/O HTN & DM. Both eyes BCVA was 6/9, N6. Right eye Fundus suggestive of Inferior Hemi-retinal vein occlusion with macular edema, confirmed by SSOCT. Left eye was normal. She opted to have intravitreal Ranibizumab elsewhere & reported us on 4th post injection day, with BCVA of 6/12, pain, redness, Vitritis and perivascular sheathing & exudation along the inferior occluded hemi-central vein. Diagnosed as FPIIE, within 2 hours of presentation; she underwent Vitrectomy with IntraVitreal Vanco, Cefta & Dexa. Vitreous sample showed Staph. Hemolyticus. Postop medications constituted intravitreal & topical Vancomycin, Topical Steroid, and Cycloplegic. She recovered a BCVA of 6/6 with clearing of retinal hemorrhages and exudates at 6 weeks.
