INTRODUCTION: Rhabdomyosarcoma is a type of sarcoma made up of cells that normally develop into skeletal muscles. Well before birth, cells called as rhabdomyoblasts –which will eventually form skeletal muscles, begin to form. Alveolar rhabdomyosarcoma often occurs in large muscles of the trunk, arms and legs. Alveolar rhabdomyosarcoma is rare in older adults. A 41 Year old female came to eye OPD with the complaints of swelling in the left eye for the past 20 days. H/o pain, diplopia present. O/E restriction of left eye movement present. Biochemical investigation showed elevated AST. CT ORBIT showed soft tissue mass suggestive of pseudo tumor. MRI showed soft tissue lesion. Biopsy from orbital mass showed malignant small round cell tumor ,s/o alveolar rhabdomyosarcoma. IHC confirms desmin and myosin. T/t excision of the mass was done followed by chemotherapy. The mass was examined histopathologically which is s/o malignant round cell tumor.
