Abstract
Purpose: This study aims to evaluates upper eyelid myectomy for blepharospasm with apraxia of lid opening (ALO) refractory to botulinum toxin.Methods: Non-comparative interventional case series. Patients had botulinum toxin-resistant blepharospasm with ALO. Myectomy included excising orbicularis oculi, depressor superciliaris, procerus, and corrugator. Primary outcome includes disability questionnaire improvement and secondary outcomes includes improvement in dry eye status, photophobia, post-surgical botulinum toxin reduction. Assessments done at pre-op, 1 month, and 1 year.Results: Included 12 patients (24 eyelids). All showed significant disability questionnaire improvement. 50% achieved post-op spasm resolution; 33.33% showed reduced botulinum toxin use. Patients with pre-surgery dry eyes and photophobia improved significantly .Conclusion: Upper eyelid myectomy effectively manages botulinum toxin-resistant blepharospasm associated with ALO, enhancing patients' quality of life
