FP1687 : Fungal Granulomas of the Orbit, the Dilemmas of Diagnosis and Management

Abstract

To report a series of 4 cases. None of the patients had any respiratory complaints or sinus tenderness.
All 3 patients underwent an incision biopsy with debulking of the lesion. With a proper surgical technique, the biopsies came positive for a fungal etiology. Aspergillosis was the causative organism,
The first patient showed a good response to oral Voriconazole as expected. The second patient needed retrobulbar injections of voriconazole for complete resolution. The third patient had a suboptimal response to local as well as systemic therapy. Multiple debulkings and local injections failed to completely resolve the growth completely and leave behind residual proptosis. An antifungal sensitivity helped in targeted treatment and achieving a better outcome.

Conclusion :
This presentation highlights the importance of Antifungal sensitivity testing and highlights available options for management of fungal aspergillosis.
It is important to customize our treatment to each patient.
