Abstract
A 50yr male patient, presented with a failed, scarred graft in his RE. In the left eye, a previously healed scarred & vascularized corneal perforation with iris incarceration, along with a mature cataract, posed complex issues. To address these problems, he underwent an Optical Triple Procedure in the LE.
The surgical intervention included the transplantation of an 8/8.5mm graft, which necessitated the release of synechiae in the eye. During the procedure, a hyper-mature cataract was unexpectedly discovered and effectively managed. A 3-piece intraocular lens (IOL) was implanted. To secure the graft, 16 sutures of 10-0 nylon were meticulously placed. The immediate post-operative outcome was highly encouraging, as the patient's vision significantly improved from HM to 6/18 within one week.
This case exemplifies the successful resolution of a complex ocular condition, where the use of PKP, IOL implantation, and precise suturing techniques resulted in a remarkable enhancement in vision.
