Abstract
Study Design: Retrospective case series
Purpose: To study the clinical presentation, microbiological evaluation, and management.
Methods: 16 patients with fungal corneal tunnel infection after cataract surgery were managed. The clinical features, laboratory work up and different management outcomes were analysed.
Results: Median interval to onset of symptoms after cataract surgery was 11.5 days (range:1-90 days). Corneal infiltrates with endophthalmitis was present in 87.5% eyes. Microbiology revealed 53.3% positivity; Candida glabrata (n=1), Aspergillus fumigatus (n=4), PCR pan-fungal positivity (n=3). All patients received 0-5 (mean 2.4) intracameral and 0-5 (mean 1.4) intrastromal injections of amphotericin B and voriconazole. The mean time of resolution of infiltrates was 9.7 weeks (13 patients), 1 patient lost to follow up and 2 patients went in phthisis.
Conclusion: Post cataract surgery fungal tunnel infections are a diagnostic as well as therapeutic challenge.
