Abstract

Deep anterior lamellar keratoplasty(DALK) is a surgical procedure to remove the corneal stroma and replace it with donor stromal tissue. Various techniques of achieving the stromal dissection has been described out of which the most popular is big bubble technique. Injection of air in corneal stroma can lead to either formation of type 1 bubble or type 2 bubble or can just lead to stromal emphysema without formation of any bubble. Chances of successful completion of DALK is more when type 1 bubble is achieved. In the event of formation of type 2 bubble, there are high chances of perforation leading to conversion of DALK to penetrating keratoplasty. This video demonstrates the way in which we can proceed in an event of formation of type 2 bubble, preventing the descemet perforation and thus successfully complete the DALK.
