Abstract

75 /M , farmer gives history of low vision in the LE since 10 years after trauma. Therapeutic keratoplasty was done for fungal keratitis 7 months back in the RE. Intra-operatively intumescent cataract was removed and posterior capsule was intact.He was maintaining functional vision with glasses for 5 months after which the visual acuity reduced .Right eye had failed graft with central corneal thickness of 1.2mm ,round reacting pupil. PC status could not be assessed and vitreous was anechoic.Considering the one eyed status, low vision , advanced age and failed graft we planned the patient for endothelial keratoplasty with secondary IOL. The video highlights the use of endoilluminator intra-operatively in complex cases with poor visibility and how it helped us place the IOL as well as the DSAEK lenticule. Post-operatively the VA improved to 20/250 and lenticular was well attached.
