Abstract
We present the surgical technique of management of microspherophakia secondary to homocysteinuria in a 4-year-old girl. She presented with a history of difficulty seeing television from a distance and a progressive increase in myopia. On slit lamp examination, she had a small, spherical crystalline lens with a visible equatorial lens edge and increased anteroposterior diameter. She underwent capsular tension segments assisted phacoaspiration with in-the-bag intraocular lens implantation and had an excellent anatomical and functional surgical outcome.
