Abstract

Major causes of a sub-macular hemorrhage are Neovascular AMD, PCV & retinal macro aneurysms. Natural sequelae when left untreated includes macular scar & sub-retinal fibrosis.Various management options are there which includes intravitreal anti VEGFs, pneumatic displacement with positioning & vitrectomy with sub-retinal tpa or combination of these. In this video we demonstrate a technique to surgically remove this hemorrhage from the sub retinal plane. Technique involves making a smiley shaped posterior retinectomy at the maximum height of the hemorrhage, away from the fovea. After this, the cutter is insinuated in the subretinal plane via this retinectomy and then the whole haemorrhagic complex is engaged using high vacuum and gradually evacuated in toto. This allows for immediate flattening of the macula and early rehabilitation of the patient.
