Abstract
65 year old woman came with complaints of redness, watering, irritation in left eye due to thorn injury. A paracentral thorn with near total corneal stromal infiltrate was noticed. Culture sensitivity, gram staining was negative. KOH scrapings showed few fungal filaments. B scan showed moderate vitreous echoes. Thorn removal with I/C antibiotics, voriconazole was planned . Lenticular injury with posterior dislocation of nucleus in vitreous cavity was noted after thorn removal. Topical , oral antibiotics and anti fungals were started for the patient. Corneal infiltrate showed no improvement .Temporary keratoprosthesis with TPPV, fragmentome assisted nucleus removal and adjunct theraupetic keratoplasty was done. Intra vitreal antibiotics/ anti fungals were given. Post operatively graft showed no signs of infiltrate. Topical and oral, antibiotics, anti fungals were continued for the patient .SFIOL with optical penetrating keratoplasty is planned at a later stage for the patient .
