Abstract
Small pupils are difficult to handle during cataract surgery. Hence, we use different pupil expansion devices to stretch pupil and make the task easier. There are various pupil expansion devices available and Malyugin ring was chosen for my case of small pupil of 4 mm in a male patient on Tamsulosin.
When the pupil failed to dilate with intracameral adrenaline/ phenocaine plus, I decided to use Malyugin ring. After the ring was inserted, a capsular tear of nearly 3 clock hours was noticed sub-incisionally. The capsule got entangled and torn after getting stuck in the scroll of the ring. This video shows how the case was managed, a good visual outcome achieved and precautions needed for avoiding such complications.
