Abstract

Study design: Retrospective analysis Purpose:To analyse neuro-ophthalmic manifestations of ADEM in children[<17years].Methods:14 Children diagnosed with ADEM who presented to a tertiary eye institution between 2018-2023 were studied.Results:The mean age was 8.71 years ±4.71 years. The male-female ratio was 5:9.The mean vision on presentation was logMAR1.152±1.025 and on the last follow-up was logMAR0.401(P value 0.001).78% had optic neuritis and 28 % had efferent involvement. Viral prodrome was noted in 57 %. MRI brain showed lesions typical of ADEM in all patients. CSF analysis showed pleocytosis in 3 patients and OCB in 1 patient. 5 patients were positive for MOG antibody and none of them had NMO antibody. Relapse was noted in 35 % (n=5) out of which 3 were MOG antibody positive.Conclusion: ADEM associated with vision loss or diplopia is a distinct clinical entity. It can be monophasic or recurrent. One-third were positive for MOG antibody. The visual prognosis is favourable.
