FP0529 : Actinomycotic lacrimal canaliculitis: A rare underdiagnosed case report.

Abstract

Purpose: Canaliculitis is a rare & often misdiagnosed case as it mimics other ocular conditions. Case: 40 yr, F presented with watering & discharge from LE last 1 yr. Her BCVA in BE was 6/6. RE was WNL. LE showed swelling & mild erythema of UL with pouting punctum and yellowish granular discharge from upper punctum on ROPLAS. Lower punctum was WNL. Lacrimal syringing patent from lower punctum, Rest examinations WNL. On gram staining showed gram positive cocci Enterococcus & filamentous branching gram positive bacilli s/o Actinomyces israelii . So diagnosis of left upper chronic canaliculitis was made and systemic & topical antibiotics was started but symptoms didn’t resolved so patient underwent for canaliculotomy + curettage. Intraoperatively yellow granular cheesy material like sulphur granules was expressed from left upper punctum. Patient was put on oral & topical azithromycin. On follow up patient showed dramatic improvement.
