Abstract

A 58year old diabetic female presented with chronic right sided epiphora and a hard mass near medial canthus. On examination visual acuity was 6/6 OU, with mild telecanthus and an ill defined, indurated non tender mass at the medial canthus of eye, extending to the bridge of the nose. Syringing of the right eye revealed partially patent drainage system with some clear fluid regurgitation. CECT showed extraconal orbital involvement with erosion of surrounding structures. Lacrimal sac incisional biopsy was done suspecting ethmoidal malignancy, histopathology of which demonstrated fungal hyphae suggestive of aspergillosis. Patient was started on systemic Voriconazole. Decrease in the size of mass and resolution was noted on follow up.
