Abstract

Purpose- Describe cases with diverse aetiology, initially suspected to have endogenous endophthalmitis (EE)

Method- Electronic medical records of cases diagnosed provisionally as EE

Result-
51-year-old male presented with EE post stick injury. LE fundus showed lesion with headlight in the fog-appearance, turned out to be sub internal limiting membrane bleed during pars plana vitrectomy (PPV)

Another patient with alike picture turned out to be polypoidal choroidal vasculopathy after PPV

patient presented with disc edema, vitritis, and retinitis lesion.
Optical coherence tomography (OCT) had subretinal fluid and outer retinal folds, which on further blood investigations was reactive for syphilis

A female who was negative for HLA-27 by conventional methos presented with clinically EE, HLA B27 repeat analysis by PCR method was eventually positive.

Conclusion-
Unusual presentations of EE might give a false imprint and lead to misdiagnosis and management in clinical practice.
