FP2570 : Case report: Aqueous misdirection syndrome in contralateral eye post routine cataract surgery

Abstract

A 62 years old male patient complained of diminution of vision in LE on the 1 st post op day without any significant pain.Surgery was uneventful/routine cataract surgery except surgeon noted extra deep AC while operating so surgery was done by reducing bottle height.He gave strange complaint of diminution of vision in RE soon after he got operated for LE cataract surgery. RE was operated 3 weeks back with routine post operative findings and UCVA was 6/6.
UCVA in LE was 6/18 and that in RE was 6/24 on 1 st post op day.IOP was 18 & 19 respectively.Myopic shift was noted as AR was showing readings -2.25 DS in BE. BE showed improvement to 6/6 with -2.25DS correction. BE showed shallow AC, LE showed +1 cells in AC. Fundus examination (IDO) of BE was normal, OCT macula reports BE were normal. B scans BE were normal. Patient was given IV mannitol stat ( there was instant deepening of AC) & was started on oral acetazolamide, aqueous suppressants, cycloplegic for a week.
