Abstract
AIM To evaluate effectiveness of vitreous surgery with direct CRA massage after two weeks of presentation
METHOD A detailed history and complete examination of a 58 year old lady referred with CRAO of left eye was done. Surgical intervention with phacoemulsification & IOL implantation, PPV & central retinal artery massage was done.
RESULT BCVA in RE was 6/6, LE was reduced to hand movement. There was RAPD in LE. NCT was 14 and 20 in RE & LE respectively for which intravenous mannitol was infused. Fundus showed cherry red spot & pale, ischaemic retina. After surgery with cataract removal, vitrectomy at low pressure & central retinal artery massage with silicon tip, emboli was dislodged and partial restoration of blood flow in obstructed artery seen. LE peripheral vision improved to 3 feet.
CONCLUSION CRAO is an ophthalmic emergency needing prompt evaluation and intervention to restore vision. Though presented late, peripheral vision improved following PPV and CRA massage.
