Abstract
We present a case of 48 years old female who came with complaints of forward protrusion of right eye for past 3 months associated with mild discomfort and pressure around the affected eye. The patient exhibited isolated RE proptosis along with conjunctival congestion.The patient denied symptoms of double vision, pain and changes in vision. Extraocular movements were full and without restrictions and IOP was 24.4 mmhg in the right eye. T3, T4 and TSH were within euthyroid range whereas , Anti TGAbs was found to be raised. Imaging studies revealed muscle involvement with optic nerve displacement. Systemic steroids was started along with topical steroid and antibiotic combination and IOP lowering agents. The presentation of TED in a euthyroid patient especially when it appears unilaterally poses a diagnostic conundrum. A high index of suspicion should be maintained for TED when evaluating patients with unexplained unilateral proptosis even in the absence of thyroid dysfunction.
