Abstract
Pseudoplateau iris(PPI) syndrome: anterior displacement of iris insertion secondary to ciliary body cysts leading to narrow angles. We present 2 cases with different clinical picture of PPI. A 40 yr lady(P1)with BE narrow angles,plateau-iris like configuration, 360° PAS on gonioscopy, raised IOP:asymmetric glaucoma (L>R),RE nuclear sclerosis II, posterior synechiae, iris bombe-complicated uveitic cataract,LE absolute glaucoma. A 59 yr man(P2) with BE narrow angles,plateau-iris like configuration on gonioscopy,normal IOP. Ultrasound biomicroscopy(UBM) for P1:RE anteriorly placed ciliary body narrowing AC, LE:PAS in all clock hours, both eyes:small CB cyst whereas for P2:BE:thick lens pushing iris anteriorly,Multiple tiny CB cysts.PPI syndrome in P1 had features of complicated uveitic cataract(pull),absolute glaucoma and P2 had BE angle closure glaucoma with thick lens causing PPI(push) which shows 2 different spectrums. Finally,treatment was cataract surgery with trabeculectomy for IOP.
