FP1666 : Drop the Blood Pressure but not Diastolic Blood Pressure

Abstract

Purpose: An interesting case report of sudden loss of vision in Right Eye 3 days post Open CABG under GA
Methodology: A 48 year old male, a known dyslipidemic, smoker and Triple Vessel Disease admitted for CABG presented to OPD with vision loss in right eye 3 days post procedure. On examination, Visual acuity was 3/60 with visual field defects. Dilated fundus examination showed both eyes small crowded disc and right eye upper 1/2 optic disc edema and splinter hemorrhages in superior disc region suggestive of Anterior Ischemic Optic Neuropathy; treated with eye drops Nepafenac 0.1% and antioxidants.
Results: Vision improved with best correction to 6/6 (Snellen's), edema subsiding with resolution of splinter hemorrhages, residual visual field defects and initial stages of optic atrophy.
Conclusion: AION may be caused due to fluctuations in BP during intraoperative period and low perfusion of Optic Nerve Head precipitated by excess post operative blood pressure correction.
