FP1347 : Standing the test of time: Cutler- Beard, reared and revered

Abstract

Purpose: To demonstrate functional and aesthetic outcome of the standard modified two-staged Cutler-Beard reconstruction (CB). Methods: Retrospective review of 48 consecutive patients who underwent modified CB for central and non-central defects. Stage I: separation of anterior and posterior lamellae of lower lid sliding flap, engaging levator palpebrae superioris in posterior lamellar sutures; and fixing height of the flap. Stage II: differential splitting of lamellae so that upper lid conjunctiva stands proud of its anterior lamella and secured with fibrin glue. Results: Mean age was 56 ± 16 years. Surgical indication included carcinoma (sebaceous, basal cell, squamous cell and Merkel cell), coloboma (congenital or traumatic), melanoma, lymphoma and xanthogranuloma with good functional and aesthetic outcome. Lateral tarsorrhaphy was needed in 1, symblepharon release in 3 and none required ptosis or eyelid surgery. Conclusion: Modified CB is effective even without tissue augmentation.
