Abstract
Purpose: In Proliferative Diabteic Retinopathy (PDR) the membranes and fibrous proliferations due to the neovascular proliferations are extremely firm and obscure the anatomy. The plane for dissection are very difficult to identify. Hence to perform the surgery with a disc onwards dissection should be done
Method: 1.Preoperative intravitreal bevacizumab injection 2.Core vitrectomy is performed to clear the vitreous haemorrhage and relieve the anteroposterior traction
3. Tug and Pull posterior vitreous separation -the posterior hyaloid along with the fibrovascular tuft around the peripapillary region is engaged with a forceps
4.Cutter delamination – a 23 G /25 G cutter at high speeds is insinuated in the PVD space and inside – out delamination is performed to feed the membranes into the port.
5.Fluid air and Silicone oil exchange-flute needle assisted FGE is performed after achieving
haemostsasis followed by endolaser and silicone oil or a gas tamponade is used
