FP2220 : Navigating Optic Neuritis Secondary to Breast Cancer Treatment

Abstract

Study Design: Case Report
Purpose
This study aimed to investigate sudden optic neuritis in a patient with breast cancer undergoing chemoradiation
Methods
A middle aged female presented with a sudden decrease in vision for one day, with pain on ocular movements. Known case of breast cancer post surgery, on chemoradiation along with gabapentin. Examination revealed reduced color vision, contrast sensitivity, and visual acuity with RAPD and disc edema. Non-specific field defects on HVF seen with T2 hyperintensity and retrolaminar diffusion restriction of optic nerve on CEMRI. Anti MOG IgG antibodies were strongly positive
Results
She was treated with a pulse of intravenous methylprednisolone for five days, followed by oral prednisolone 1mg/kg OD which led to the resolution of symptoms within one week
Conclusion
On follow-up, visual acuity, color vision, and contrast vision improved. The pupils regained their reaction to light and the disc edema resolved. A visible Paton's line was noted
