FP1042 : Third nerve palsy : Medial Rectus – Resection versus fixation to Medial Palpebral Ligament!

Abstract

To analyse and compare the outcome of two different surgical procedures in patients with complete third nerve palsy with large angle exotropia.
This is a retrospective analysis of 35 patients with total third nerve palsy and large angle exotropia from January 2006 to June 2020 who underwent either of these two surgical procedures.
Group 1: Lateral rectus deactivation with medial rectus resection.
Group 2: Lateral rectus deactivation with medial rectus fixation to medial palpebral ligament (MPL).
Surgical alignment on the first post-operative day, and at 1 and 6 months postoperatively, were analysed, with postoperative complications. All statistical analyses were performed using STATA version 14 and P value <0.05 was considered significant.
Surgical success was greater with group 2 (93.33%) than group 1 (65%). Postoperative exotropic drifts were noted in both the procedures but seen more with patients in Group 1. However, postoperative complications were noted only in Group 2 patients.
