Abstract

PURPOSE
To discuss a case of Acute exudative polymorphous paraneoplastic vitelliform maculopathy misdiagnosed as central serous chorioretinopathy.
METHODS
32 yr old male presented with metamorphopsia in RE since 6 weeks.On examination RE had visual acuity of 6/9 with multiple neurosensory detachments in posterior pole. Fluoroscein angiography showed staining of PED with no evidence of leakage, hence labelled as Chronic CSC. After being lost to followup for 2 years , he presented with complaints of fluctuating vision in RE. He had undergone left orchidectomy 1 year back ,and HPE showed Seminoma . He also had a left paraaortic mass which turned out to be Seminoma on FNAB.
RESULTS
EOG showed a Arden’s ratio of 1.3 in RE and in 1.9 in LE.A diagnosis of AEPPVM secondary to metastatic seminoma of testis was made.There was complete resolution with chemotherapy and systemic steroids.
CONCLUSION
AEPPVM is a rare condition that needs a high index suspicion for early and accurate diagnosis.
