Abstract

A 70-year-old male presented with sudden vision loss in the left eye following cataract surgery. The examination revealed pseudophakia in both eyes. Posterior segment examination showed retinal abnormalities, including perivascular infiltrates, miliary retinitis, and macular edema in the left eye. OCT examination confirmed hyper-reflectivity of the inner retina and cystoid macular edema in the left eye. Fluorescein angiography revealed disc leakage, perivascular sheathings, non-perfusion areas in both eyes, retinal vascular occlusion not respecting any typical pattern and macular edema in the left eye. Initial investigations (total count, differential count, ESR, Mantoux, ANA, Anti DS DNA, c-ANCA, p-ANCA, APLA) yielded normal results, but syphilis screening was positive. The treatment involved intravenous antibiotics, oral steroids and topical steroids. Follow-up visits showed improved visual acuity and resolution of macular edema.
