Abstract

A 63-year male came to opd with complaint of inward displacement of left eyeball since 1 year and diminution of vision LE since 6 months. The LE displacement was gradually progressive associated with diminution of vision since 6 months, and watering, redness since 2 months . H/o trauma to LE with wood while working in fields 1 month back after which the vision gradually came down to PL-ve. There was no h/o diplopia, pain, headache, weight loss. On examination, there was anteromedial displacement, with left upper eyelid covering more than half of cornea (6 mm) with vertical dystopia and phthisis bulbi of left eye. On palpation there is 3 x 2 cm smooth, firm, non-tender and immobile mass. The eye is in depressed and adducted position. EOM are partially restricted in all positions of gaze. MRI showed well defined solid cystic lesion in the extraconal space of left orbit s/o lacrimal gland neoplasm. Exenteration was done and mass sent for HPE diagnosis of pleomorphic adenoma was made.
