FP1769 : Total Graft Avulsion During suture removal – Rarest of Rare Distaster

Abstract

Purpose:- To report a rare case of disastrous complication during corneal graft
suture removal.
METHODS: 51 year old female operated for optical penetrating keratoplasty with
cataract surgery following healed fungal corneal ulcer. On 8 months follow up, selective suture removal done. Superotemporal two sutures removed, sudden gush of fluid noted. Immediately resuturing was done. Later suture removal was planned after 6 months. Selective suture removal was successful, improving BCVA to 6/24. On last visit, 4 remaining
sutures (same superotemporal area) were removed. Mild thrust was given to last suture's stuck knot. Patient suddenly moved back and whole graft
avulsion happened.
Immediately graft resuturing was done. BCVA
decreased to hand movements. Ac was formed. PCIOL
was in intact.
CONCLUSION: Wound healing in the present case has
not been probably completed earlier , therefore
extensive wound dehiscence may be observed even
after minimal mechanical force
