Abstract
Many times while operating on an intumescent cataract we encounter argentian flag sign while doing curvilinear capsulorhexis and it becomes risky to continue with phaco emulsification as there may be posterior extension of rhexis which may lead to nucleus drop. so in such cases it is important not to further lead to raised IOP and manual Small incision cataract surgery comes to rescue.
One such video of a intumescent cataract surgery where in there is argentian flag sign and there is no way to rescue the extended capsule, so the surgeon immediately converts in to capsulotomy and proceeds with msics and completes the case with iol implantation.
