Abstract

A 33-year-old female presented to the OPD with diminution of vision for 15 days in the right eye. On examination, her general and systemic examination within normal limits, ocular examination- visual acuity of the right eye: finger counting up to 1 foot and left eye: 6/6. The right eye: posterior synechiae with no signs of active uveitis; dilated fundus examination (right eye): subretinal abscess with overlying exudative retinal detachment involving the macula. Left eye examination: not significant. Vitreous biopsy from right eye sent for TB-PCR: positive for Mycobacterium tuberculosis. The patient started on ATT as per RNTCP for 9 months (Isoniazid, Rifampicin, Pyrazinamide, and Ethambutol). One week later systemic steroids (1mg/kg) started. Resolution of subretinal abscess and chorioretinal scarring noted. Vision in the right eye improved to finger counting from 4 feet. Conclusion: timely diagnosis and early treatment can prevent vision loss and systemic complications due to TB.
