VT0419 : Stitching Vision Together: CMTFlex Foldable Secondary IOL-A Sewing Machine for Iridodialysis Repair

Abstract

A 59yr/F presented with large iridodialysis of 5 clock hours & surgical aphakia post RE SICS with diminution of vision, photophobia. BCVA was HMCF, IOP 19mmhg. After initial medical management of 6weeks, patient was planned for pupilloplasty & secondary IOL under LA under guarded prognosis. Conjunctival peritomy with bipolar cautry was done. Partial thickness scleral flap made at 0 & 180 degrees from limbus. Iris repaired using sewing machine technique,creating a scleral groove parallel to the damaged portions of the iris, suture loops made along dialysis, which then passed through the iris from inside out. The loops cut and tied, the knots buried under scleral groove. clear corneal entry made using 3.2 keratome and CMTflexfoldable lens using PraNiv T Flex forcep & Nishi grasping forcep fixated. Sclerostomy opening made with 23G MVR. Haptic brought out of 0 & 180 degree sclerostomy site. Fibrin glue put to reposit the conjunctiva. Postop BCVA 6/18 with circular pupil, no photophobia
