VT0322 : The story of a CRIPPLED IOL

Abstract

A 50-year-old female underwent cataract surgery using phacoemulsification at a clinic specializing in anterior segment surgery. Unfortunately, a posterior capsular rupture occurred during trenching. The operating surgeon documented this event and referred the patient to our institute's Vitreoretina department. The following day, we took on the case, which involved remanent cataract removal, vitrectomy, and the placement of a secondary intraocular lens. During the anterior vitrectomy and removal of the remaining cortex, we discovered a subluxated, foldable IOL hanging via a broken haptic. We successfully removed this IOL from the vitreous cavity and placed a 3-piece IOL in the sulcus. This case underscores the management of a complex cataract, and emphasizes the importance of surgeons being transparent when referring cases to their colleagues and the medical community, ensuring that such cases are effectively and professionally handled.
