Abstract
This video demonstrates the use of a combination of old, new and some innovative techniques, to tackle two difficult surgical case scenarios. The first case pertains to a 30 years old blind patient with traumatic retinal detachment with retinal incarceration, approaching closed funnel configuration. Careful utilisation of external support to the incarceration with encirclage, external drainage of subretinal fluid before port making and careful release of incarceration internally with the cutter in moment mode, brought a new lease of life for the patient with a best corrected visual acuity of 6/12.
The second case is about a young patient with macular tractional retinal detachment and a seemingly inoperable crumpled posterior pole, where careful removal of all membranes results in a good anatomical outcome. A combination of all available techniques, with the use of a cutter, helped obviate the need for bimanual surgery, as well as reduced the chances of iatrogenic break formation.
