Intraoperative Floppy iris syndrome(IFIS) remains a challenge for surgeons during phacoemulsification. Lack of awareness can turn a routine, uneventful surgery into one with significant visual morbidity. Use of tamsulosin, an alpha adrenergic receptor blocker used in benign prostatic hyperplasia was initially incriminated in it’s etiology , but later other alpha adrenergic receptor such as alfuzosin, terazosin and doxazosin alongwith different other class of medications and systemic risk factors were identified. A triad of intraoperative signs , varying in degree , classically characterise the IFIS – Billowing of the Iris , Iris prolapse at the incisions and intraoperative miosis. A good preoperative assessment and Prophylactic measures need to be in place, with some surgical modifications and intraoperative management.Increasing awareness about IFIS , knowing what troubles to anticipate , is the key to better management
