Abstract
Study design:- Retrospective observational
Purpose:- Clinical outcomes of deep stromal fungal keratitis (FK) following anterior chamber (AC) wash with intracameral and intrastromal (IC and IS) Voriconazole.
Methods:- Demographics, risk factors, clinical course, microbiology, and visual outcomes of patients with deep-seated FK who underwent AC wash with IC and IS Voriconazole were analysed.
Results:- Mean age of 40 patients was 53±12 years, and mean time to presentation was 12±8 days. 52.5% of cases exhibited severe grade ulcers. Aspergillus flavus was the most isolated organism, 16(40%). Of the 7(17.5%) patients who achieved complete resolution with scarring, 6(85.7%) patients had moderate grade ulcer and 3(42.8%) were due to Fusarium solani. 33(82.5%) progressed to Therapeutic keratoplasty (TPK) and 10(30.3%) had reinfection.
Conclusion:- AC wash with IC and IS Voriconazole is an ineffective adjuvant therapy in cases of deep-seated fungal ulcers. Early TPK may have a better outcome.
