Abstract

Microspherophakia is a rare congenital condition where the crystalline lens has a spherical shape with an increased anteroposterior distance and reduced equatorial diameter.
A 30 yrs old male presented with RE ocular pain. IOP of RE was 46.5 mmhg and LE 20.7 mmhg. Slit lamp examination showed anteriorly dislocated microspherophakic clear lens in BE with circumcilliary congestion and corneal edema in RE. BE fundus were WNL. Systemic examination revealed short stature and stiff joints. Pt was given injection mannitol and oral acetazolamide for 2 days which subsided corneal edema, improved VA from FCCF to 6/60 BCVA and RE IOP to 20 mmhg. Pt was planned for RE extracapsular clear lens extraction, anterior vitrectomy and iris claw implantation.Post operatively pt's VA improved to 6/18 BCVA and RE IOP was 14 mmhg. After 5 days pt followed up with same presentation in LE. Similar medical and surgical approach was followed for LE. Post op VA of LE was 6/18 BCVA.
