FP0397 : Single stage, one eye, ALL six muscles squint sugery for ultra large angle squint.

Abstract

Study design: Case report. Purpose: To demonstrate the feasibility of single stage squint surgery on all six extraocular muscles using the ciliary vessel sparing technique (CVST). Methods: A 17 year old female with -86PD exotropia and +50PD hypertropia in the right eye underwent Lateral rectus recession – 12mm using an adjustable suture, Superior rectus (CVST) recession – 6.5mm, Inferior rectus (CVST) plication – 7mm, Medial rectus (CVST) plication – 8mm, Superior oblique translational recession – 10mm and Inferior oblique recession – 14mm & anteriorization – 4mm. Result: Postoperatively from 21 days till date (3 years), excellent and stable ocular alignment (7PD esotropia and 4PD hypertropia) was obtained without any signs of anterior segment ischemia. Iris fluorescein angiography showed a blocked fluorescence due to masking by the iris pigments. Conclusion: A meticulus, ciliary vessel sparing technique (CVST) allows safe and effective single stage six muscle repair.
