Abstract

Purpose: To report a rare complication of Optic neuritis in a case of HZO. Case: 33-year-old male presented with sudden visual loss in his right eye for 2 days. Two weeks earlier, his right eye was diagnosed with HZO, and he received oral acyclovir for 10 days. His visual acuity was 6/36 OD and 6/6 OS with RAPD in the right eye. Hypoesthesia was noted in the area of the HZO. A clear cornea with a quiet anterior chamber was noted. Optic disc appeared healthy without any signs of papillitis. A clinical diagnosis of HZO-related right retrobulbar ON was made, and other causes of atypical ON were excluded. CEMRI brain and orbit revealed increased optic nerve sheath diameter on the right side. VEP confirmed right anterior visual pathway lesion. He received methylprednisolone as per ONTT. After the completion of treatment, the visual acuity recovered to 6/9 in the right eye. However, inferotemporal visual field defect was noted. Conclusion: Isolated ON is a rare complication following HZO.
