FP2952 : Keeping an eye out for post fever retinitis

Abstract

Post-fever retinitis (PFR) is an infectious or para-infectious uveitic entity caused by bacterial or viral agents and seen mainly in tropical countries like India.
A 34 year old male presented with gradual progressive painless diminution of vision in both eyes since 10days which was preceded by fever lasting for 3 days. Visual acuity was 6/60 in both eyes. Fundus showed macular thickening associated with macular star pattern in both eyes.OCT suggested B/LSubretinal fluid suggestive of NSR detachment.On investigation, Weil Felix OXK was strongly positive. A diagnosis of retinitis secondary to Post rickettsial etiology was made. Patient was started on T doxycycline & steroid
There was improvement in the visual acuity and he regained 6/9 vision at the end month.Retinitis on posterior pole with recent history of fever and a positive Weil-Felix test may suggest a rickettsial etiology.Although it has aggressive presentation, it has a good visual prognosis.
