Abstract
A 65 year old, obese female presented to the emergency department with complaints of fever, chills, shortness of breath and altered sensorium. She was a known case of hypertension and obstructive sleep apnea. She was managed with IV fluids, broad spectrum antibiotics. antipyretics and oxygen supplementation.
She developed respiratory failure, respiratory acidosis, kidney failure and sepsis – both blood and urine cultures were negative.
The Ophthalmology team was called as a small amount of discharge was observed around her left eye. On examination, it was found that she had an ocular prosthesis in her left eye socket with erythema over left eyebrow. On removal of the prosthesis, copious amounts of pus oozed out from the socket which was sent for culture – A nidus of infection was finally found. The pus culture report showed Methicillin-Resistant Staphylococcus Aureus – sensitive to Linezolid which was started immediately. The patient's response was terrific and her life was saved.
