Study Design: Case Report
Purpose: To study approach and management in a case of Adie’s pupil
Methods: 24 y.o M came to ER c/o blurring of vision in right eye (RE) since 5 hours. Patient has no h/o drug use, ocular trauma but gave h/o suffering from Varicella zoster 10 months ago. On torch light examination, his RE was 7 mm in size, irregular in shape & NRTL. The other pupil was 3mm, RTL. In a lit room, anisocoria was greater. Near reflex was present. EOM were normal. On slit lamp exam, no signs of inflammation were noted. The fundus was WNL.
On administering 0.125% pilocarpine eyedrop, patient’s eye constricted to 5mm & was sluggishly RTL after 15 min. On re-evaluation on slit lamp, patient showed iris sector palsy.
Result:Patient was diagnosed with Adie’s pupil a/w h/o varicella zoster in the past year & given 0.125% pilocarpine e/d TDS
Conclusion: Acute presentation of anisocoria which is more pronounced in light with no h/o trauma, drug use & normal EOM is most likely Adie’s pupil
