Abstract
Case study- A 38 year female presented with horizontal binocular diplopia for past 8 months more on the right gaze & gradually progressing for past 3 months. She had excision of pterygium nasally and temporally in left eye 1 year back. She had frequent redness and pain in left eye for 8 months.
Methods-Left eye showed enophthalmos relative to right eye, recurrence of temporal pterygium, globe retraction during right gaze with narrowing of palpebral fissure on adduction and adduction limitation. Diplopia charting showed crossed horizonal diplopia more on right gaze.
Conclusion-Plausible reasons for acquired retraction include post operative conjunctival / tenons scarring temporally or a tight lateral rectus syndrome or medial rectus injury leading to lateral rectus tightness and subsequent medial rectus contracture causing retraction or a reverse leash effect of recurrent pterygium.
