Abstract
A 44-year male, diabetic, Hypertensive- 10 years present with bilateral diminution of vision. Both eyes visual acuity PL. Anterior segment examination IOP was normal. Fundus examination revealed pale disc, retinal haemorrhage and laser chorio-retinal atrophy in Right eye and traction retinal detachment (TRD) involving disc and macula, SRF and sub retinal gliosis in left eye. OCT right eye showed Maintained foveal contour, left eye had traction involving macula. He was further advised for MRI brain and orbit for right eye disc pallor. MRI brain and orbit showed large infarct in left MCA territory and right PCA territory. He was advised for left eye Vitrectomy, membrane peeling, endolaser with tamponade. All membranes were dissected meticulously without any iatrogenic break, so SRF not drained intra-operatively. Silicon oil was used as tamponade. Post-operatively macula was attached with shallow Subretinal fluid, however vision improved to finger count 1 m in left eye.
