Abstract

Study Design-Retrospective study in 198 eyes over 3 months. Purpose-1. Evaluate degree of agreement of Van Herick's(VH) grading done by different ophthalmological subspecialists. 2. Examine accuracy of VH grading by in comparison to gonioscopy. 3. Establish safety of mydriatic examination without gonioscopy. Methods-Patients with VH grade </= 2 referred to glaucoma clinic. VH redone by glaucoma specialist to confirm. Gonioscopy performed in all patients(Shaffer classification used). Statistical analysis using Student T-test. Results-24 out of 102 eyes initially classified as VH Grade 2 reclassified to Grade 3(Error rate=12.12%). Agreement between VH & gonioscopy=93%. Sensitivity of VH=95.5%, specificity=91.7% (p=0.02). Conclusion-VH method is a reasonably accurate predictor of narrow angle; high degree of correlation with gonioscopic grading. However, gonioscopy remains gold standard. Safety of mydriatic examination can be judged with reasonable certainty on the basis of VH method.
