Abstract

A 29 year old female visited OPD with complaints of sudden painful diminution of vision, proptosis of the left eye following accidental blunt trauma with a mango tree branch 3 days back.In LE, visual acuity was reduced to hand movements close to face, restricted extraocular movements, diplopia, 4.5mm proptosis; no other ocular/systemic abnormalities were noted. A CT guided minimally invasive orbital procedure was undertaken where a 20Gz needle was advanced transcutaneously into the superior orbit and 14 ml of blood was aspirated. Proptosis resolved immediately. On POD1, visual acuity improved to 6/12 with normal ocular motility. This procedure provides excellent immediate outcome, averts the need of exploration and drainage by orbitotomy or endoscopic surgery; early rehabilitation, no risk of post operative complications like CSF leak, meningitis.
